As you push through Diablo 4's challenging endgame content—whether it’s Nightmare Dungeons, Helltides, or World Tier 4—the importance of gearing up with purpose becomes crystal clear. While loot rains from slain enemies and chests alike, not all gear is created equal. At high levels, especially past level 70, the quantity of loot increases, but your need for precision and synergy becomes more critical than ever.

So, what gear is actually worth equipping at high levels in Diablo 4? This guide will break it down: from what affixes to prioritize, to whether Legendaries or Uniques reign supreme, and how to distinguish great gear from clutter.

1. Focus on Stat Synergy, Not Just Item Power
At first glance, Item Power may seem like the most important metric, but at high levels, what truly matters is how the affixes support your build. For example:

Critical Strike Chance and Damage are essential for crit-based builds like Rogue and Sorcerer.

Lucky Hit Chance is crucial for triggering effects in certain Druid or Necromancer setups.

Cooldown Reduction is game-changing for classes with long ultimate abilities.

+Ranks to Key Skills or Passives significantly boost your overall output.

A lower item power item with perfect affixes can outperform a higher item power item with mismatched stats. Learn which stats your build needs and search accordingly.

2. Prioritize Sacred and Ancestral Gear (World Tier 3 & 4)
